The User ID and Password you receive provides access to all three of the applications on this Document For Safety website (www.documentforsafety.org) - A+KIDS, BRANDS and Synagis. |

Attention Newly Registered A+KIDS Providers If you have a specific patient for whom safety documentation needs to be submitted prior to receiving a user ID and password, a fax option is temporarily available to meet this need. Fax authorizations are processed within 24 hours of submission. |
